Unsecured hatch, long hours and lighting caused fatal fall

THE INVESTIGATION into a fatal 12m fall at the Gladstone Queensland Alumina plant on 17 July 2005 has concluded an unsecured hatch played a part in the tragic accident.
Contractor Colin Arthur Greaves died from his injuries after falling 12m at the facility.
According to the Coroner, the hatch on settler tank number six had not been secured, and this played a part in the fall.
There were other hazards which increased the risk of a fall. It was night-time and lighting was minimal. The worker was also not protected by hard barricades or harnesses.
The contractor had been working for more than the maximum shift time of 12 hours. The Coroner found he probably tripped into the hole or forgot it was there and fell into it.
