A form worker was struck by half a steel column shutter and killed, prompting WorkCover NSW to publish a safety alert on the erection and dismantling of formwork. The steel column shutter had been left on a poured concrete column while the other side of the shutter was removed during the stripping operation. The safety alerted employers to ensure the dismantling of formwork is done in a safe manner that is planned and controlled in accordance with the 1995 regulations in regard to formwork for concrete.Workers should regularly review and amend safe work methods according to changing conditions. Employees should be provided with appropriate training and instruction, including safe work statements.Employers were also urged to ensure all parts of formwork and false-work assemblies are stable and secure at all times during the forming and stripping operations, as well as during temporary storage on site.
